Stop Leaving
Medicare Revenue Behind.
Whether your organization has not yet activated care coordination, or you are already billing CCM and missing PIN and CHI on the same patients, Premier Care Coordination is the nurse-led contracted partner that captures every concurrent dollar Medicare allows.
We deliver Community Health Integration, Principal Illness Navigation, and Chronic Care Management as nurse-led care coordinators on our own CMS-compliant software under physician supervision. You bill under your NPI. We deliver and document.
If You Already Bill CCM, You Are Likely Capturing Only a Fraction of the Concurrent Reimbursement Available on the Same Patients
The partners we work with often come to us already billing Chronic Care Management. They have done the harder operational work of building consent workflows, training staff, and operationalizing time tracking. What most have not realized is that the same patients qualifying for CCM almost always qualify for at least one additional concurrently billable Medicare program.
If a CCM-enrolled patient has a serious high-risk condition such as cancer, congestive heart failure, COPD, dementia, end-stage renal disease, severe mental illness, or substance use disorder, they qualify for Principal Illness Navigation in the same calendar month. If that same patient faces transportation barriers, food insecurity, housing instability, or social isolation that affects their treatment plan, they also qualify for Community Health Integration.
Both PIN and CHI are billed concurrently with CCM. Neither replaces it. CMS explicitly permits concurrent billing as long as time and effort for each program are uniquely documented and not double-counted. The documentation discipline this requires is exactly what most organizations do not have time to build internally, and exactly what Premier Care Coordination is purpose-built to deliver.
If your organization currently bills CCM, the question is not whether to add care coordination revenue.
It is whether you are willing to keep leaving concurrent reimbursement uncaptured every month.
