Premier Care Coordination was founded after CHI and PIN already existed, specifically to deliver the three core Medicare care coordination programs as our primary specialty.
We were founded by a Registered Nurse who saw the gap. CMS opened the door for practices and facilities to capture meaningful Medicare reimbursement for the coordination of care most clinical teams already wished they could deliver. What was missing was a clinical partner credentialed to deliver it compliantly, under physician supervision, without adding burden to the organizations that needed it most.
