About us
- Who is Premier Care Coordination?
- We are a nurse-led company helping seniors age in place. Our team of RNs, LPNs, and community health workers helps people navigate a fragmented healthcare and community system. We work with seniors and families, physician practices and hospitals, and cities and communities.
- Where are you based?
- We are based in Gary, Indiana.
- What are PCC Services, Juun, and Village Hub?
- PCC Services is our nurse-led care coordination team. Juun is the care coordination platform our team runs on, which other organizations can license. Village Hub is our community platform, one front door where residents find help.
Seniors and families
- What can you help with?
- Rides, meals, help with bills, care at home, and finding the right services. We make the connection and then follow up until the help has actually arrived.
- Do you work with my doctor?
- Yes. When there is a health need, our nurses work with your own doctor.
- Is my information kept private?
- Yes. We take privacy seriously and never share your personal or health information without your permission.
Physician practices and hospitals
- What programs do you deliver?
- Community Health Integration (CHI), Principal Illness Navigation (PIN), Chronic Care Management (CCM), and Transitional Care Management (TCM). These are Medicare programs that support patients between visits.
- How do you work with a practice?
- We become your care coordination team. Our nurse-led team works under your physicians, and your practice bills Medicare for the services.
- How do you work with hospitals?
- We help patients after they leave the hospital by working with each patient's own physician, connecting them to the help they need at home, and checking in so they stay on track.
- Do we have to replace our staff?
- No. We add capacity your team does not have time for. If you have your own care staff, you can license Juun, our platform, and run care coordination yourselves.
Cities, communities, and Village Hub
- What is Village Hub?
- Village Hub is one front door where residents find local help, connect with a navigator who follows through, and where we confirm the help was actually received.
- Who pays for Village Hub?
- Cities and communities fund it. Residents never pay.
- Does it replace our existing programs?
- No. Village Hub works alongside the programs you already have, including Area Agencies on Aging, and connects residents to them.
- What does a city get?
- Live data on what residents need and what happened next, to help guide funding and planning.
- Can we run it ourselves?
- Yes. We can run Village Hub for your city, or your city can license it and run it with your own staff.
